Your care plan is individual to you

You will be encouraged to participate in every aspect of living at Edgemont View. This includes contributing to your care plan. Every resident has a care plan which provides the basis upon which Edgemont View’s nursing care service is delivered.

The care plan includes a description of your preferred daily routine, any foods, and drinks you particularly like or dislike, any dietary requirements, what you like to wear and how you prefer to be addressed. The care plan also has details of health care needs, medication, information about your GP and any community nursing or other services provided.

Your personal profile will include details of your life history before entering the home, social interests, hobbies, and arrangements to attend religious services of your choice, all of which are intended to enable staff to understand your needs and relate to you well.

At Edgemont View, we use a digital platform that helps us build up a care plan around each individual resident and provides the basis upon which care service is delivered. Our care plans are continuously under review to ensure their care support remains appropriate for them.
